Please take a moment to fill your brief New Patient Health History Form out to completion before your first visit. This allows you to be seen on time so you can put your pain behind you.  If you do not have a printer please plan on arriving 15 minutes before your scheduled appointment so you can complete this in our office.  You can also complete this on online.  

We ask that you wear / bring gym clothes (shorts/ t-shirt) to your appointment because they are more comfortable than paper gowns and we need to examine you. We have private rooms to change clothes if needed.

Please bring any outside records related to your condition (such as x-rays, MRIs, CTs, lab work, or reports).  This prevents delays and duplicate tests so you get the relief you want sooner.

We have always been committed to the privacy of our patients. Federal regulations require all medical offices to provide full disclosure of office privacy policies. Woodward Chiropractic & Massage’s HIPAA Notice of Privacy Practices should answer any question you may have in this regard. Additional copies can be obtained from our office on your next visit.
