Most patients complete the form in about 5–10 minutes.

Please complete a new form if this is your first visit or if it has been more than 12 months since your last appointment.

Before Your Appointment

Wear or bring comfortable clothes you can move in, such as shorts and a T-shirt.
Bring a photo ID and insurance card, if we will be billing insurance.
Bring any relevant outside records, including X-rays, MRIs, CT scans, lab work, or reports.
Arrive a few minutes early if you were unable to complete your forms in advance.
